This is the document I received from my company in relation to the pre-employment medical examination. The type of examination I need is a Type III medical examination (section 5.6 of the document).

If there is any difficulty with the language of the document, could you please let me know.

If my understanding is correct you are a white collar worker and my doctors in surgery can perform this kind of medical.

Dear Dr Krasnicki
Our Firm is a UK Occupational Health company assisting our clients by arranging medical examinations internationally.

I have a client who requires a fit to travel and work medical examination for an employee who lives in Wroclaw and will travel to South Africa on 11th January 2014. The employee is available after 2nd January 2014.

I have attached the sample medical form which details the protocols to be completed. I would be pleased if you were able to assist us on this occasion and look forward to hearing from you.
